Samoa's Government has declared a state of emergency, ordering all schools, including the National University of Samoa, to be closed as a deadly measles outbreak continues to spread.

Since Samoan officials announced a measles epidemic in October, seven suspected measles-related deaths have been recorded.

The majority of cases have involved children younger than four years old.

The Samoan Government ordered children under the age of 17 not to attend public gatherings in an attempt to stop the virus spreading.

It also made vaccinations a mandatory legal requirement for all people of Samoa who have not yet received a vaccination injection.

Measles is a highly infectious airborne disease that spreads easily through breathing, coughing and sneezing. It typically causes a rash, fever and white spots in the mouth.

More serious complications include blindness, pneumonia, brain damage and severe dehydration.

It is believed the outbreak started in New Zealand before spreading to Samoa. Western Australia has also been affected with one of the worst outbreaks in years.

Neighbouring American Samoa announced a public health emergency over a measles outbreak on Thursday.

Governor Lolo Matalasi Moliga said everyone travelling from Tonga and Samoa to the US territory must provide proof of measles immunisation as a condition of entry.

Visitor who fail to provide a proof of measles immunisation will be denied entry to American Samoa.

Samoa Airways chief executive Tupuivao Seiuli said it was vital that all visitors travelling to American Samoa through Samoa comply with these new conditions to reduce further disruption to their travel plans and assist neighbouring countries in their efforts to contain the recent outbreak of measles.

In Tonga, all government primary schools and kindergartens have been shut in an effort to limit the spread of measles sweeping through the South Pacific, the Government announced on Thursday.

Education Minister Siaosi Sovaleni said schools would be closed until at least November 25.

Samoa has recorded its 3000th measles case - approximately 1.5 per cent of its population - as the crisis which has claimed 42 lives shows no signs of slowing.

On Friday morning, the Pacific nation revealed a further 213 cases had been confirmed in the past day, taking the total to 3149.

All but four of the deaths have been children under four years, with the 1-4 year age bracket recording the most cases - 985 sick and 20 deaths.

Eighteen children under 12 months have died.

The other four deaths were one each in the age brackets 10-14 and 30-39, and two between 15-19.

The 20-29 year age bracket has the second most number of cases with 475, but no deaths.


Earlier this month, the Samoan government declared a one-month state of emergency which included mass vaccination efforts.

The government says so far, 50,068 people have been vaccinated as part of the push.

It is believed the country of 200,000 had a two-out-three immunisation rate prior to the mass vaccination.

(CNN)The government of Samoa will shut its doors Thursday and Friday and redeploy civil servants to help deal with a deadly measles outbreak, Prime Minster Tuilaepa Lupesoliai Sailele Malielegaoi announced in a special address Monday.

Fifty-three people -- 48 of them children younger than 4 -- have been killed by the disease in the South Pacific island nation in recent weeks, the government said in a news release. A total of 3,728 cases had been recorded as of Monday, according to the government. None of the victims were vaccinated, Tuilaepa said.

The decision to shut government services for two days is the latest in a series of drastic steps Samoan authorities have taken to stop the outbreak of a disease that was thought to be almost eliminated globally, but has made a dangerous comeback in recent years.


Schools throughout Samoa have been shut indefinitely since November 17 due to the crisis. Authorities have not set a date for them to reopen, according to Nanai Laveitiga Tuiletufuga, the Prime Minister's press secretary. Children have been banned from all public gatherings and places where "large numbers of people congregate," the Prime Minister said.


Samoa's government officially declared a state of emergency on November 15, according to UNICEF, and began a mass vaccination campaign five days later. Tuilaepa said 58,000 people -- more than a quarter of the population -- were vaccinated from the start of the campaign on November 20 until Monday.

All civil servants, except for those who help supply water and electricity to the country, will participate in the vaccination campaign on Thursday and Friday by offering assistance to public health officials, Press Secretary Nanai said.

Measles is a highly contagious, vaccine-preventable respiratory illness characterized by a rash of flat red spots. Symptoms may include fever, cough, runny nose and watery eyes.

Measles can also lead to death when complications become too severe, according to the US Center for Disease Control and Prevention. About 1 child out of every 1,000 who get measles will develop encephalitis -- swelling of the brain -- that can lead to convulsions, deafness or intellectual disability. As many as 1 out of every 20 children with measles will get pneumonia, the agency reports.

Measles has seen a wide resurgence around the world -- in both high-income countries in the Americas and Europe and lower-income nations in Asia and Africa -- fueled in part by fear of and lack of access to vaccines, and complacency.

Almost 350,000 measles cases were reported globally in 2018, according to UNICEF -- more than doubling from 2017.

In his speech, Tuilaepa called on community leaders to "encourage and convince those that do not believe that vaccinations are the only answer to the epidemic."

"Let us not be distracted by the promise of alternative cures. Measles is not a new disease to Samoa and rarely claimed lives," he said.

Quote Families that have not been vaccinated against measles in the Pacific nation of Samoa have been asked to hang a red flag outside their homes to help fight a deadly outbreak of the disease.

The flags will assist medical teams travelling door to door inoculating residents.

The government says more than 4,000 people have been infected with measles out of a population of 200,000.

Sixty people have died so far, many of whom were children under five....

Samoan officials say the vaccination rate has now reached about 55%. Prime Minister Tuilaepa Sailele Malielegai has vowed to get the figure above 90%.

Samoa's Prime Minister says he's "angry and disappointed" at those in Samoa who continue to avoid vaccinations, even as the country is shut down on Thursday under strict emergency laws.

The public has been banned from driving on the roads, ordered to stay home and wait for vaccinators.

Newshub witnessed young people refusing injections as emergency teams went door-to-door across the country.


The day started at dawn for the masked frontline fighting against Samoa's deadly epidemic.

Even parliamentary staffers were told to cease normal duties and help.

"We are hoping that the community will make use of this opportunity," Samoa's parliamentary clerk, Tiatia Tualaulelei, told Newshub.

The entire nation of Samoa was ordered to "stay home" on Thursday. On the deserted streets, mobile teams were looking for red flags or bits of cloth, tied to posts outside homes.

For the mobile emergency vaccination team, red indicates that a family has not yet been immunised.

Police were also out in force, checking vehicles and official paperwork.


Despite the death toll hitting 62, apprehension and resistance to the campaign continues.

A young man fled from the immunisation team at one property, refusing the vital vaccine offered by Waitemata DHB public health nurse Sue Wilson.

"He ran away. He's feeling scared," Wilson said.

Samoa's Prime Minister, who was only vaccinated on Wednesday, also spoke of witnessing young people refusing to be immunised.

"It is disappointing and I am very angry. Very angry in the sense that we have done all that is possible," Prime Minister Susuga Tuilaepa Sailele Malielegaoi said.

However, progress is being made. Now, 76 percent of infants - the group who are most vulnerable - have been vaccinated.


"To see children, the young ones growing up in the country... it's tough for parents and it's tough for everyone," father Savelio Ahcolt told Newshub.

"Seeing how small babies are passing away... it's like a cyclone that comes and just crashes away all the people," said local Faafeao Saienisi.

In Le'auva'a, village representative and elder Vaifale Sili told us five children in the community had recently been buried.

"I miss all the people in this village, from this family. Why do this? We say a prayer to God... don't know what has happened," she said.

Everyone is praying in Samoa - hoping the crisis will soon be over.

I've just been reading BoredPanda's list of replies to the anti vaxer mum who complained about being called an antivaxer. The replies were hilarious!

"We were being kind, but: "crazy child killers' will do instead" was one of the milder ones. "Hate driven science regressives" was a cracker and my own favourite: "I only want a child for three years faction". I also love the simple: "Bioterrorist!"

It was brutal, but I have to say earned.

What an evil man/blythering idiot! I hope they throw the book at him.

Even his choice of vitamin is off the mark. C is involved in the immune system's response, but is ineffective without zinc and potentially attacks the host without sufficient vitamin D. These people are already malnourished; it can't work! Papaya leaf does boost the immune system, but this will not work in cases of malnourishment and can have the following really nasty side effects: severe stomach/abdominal pain, nausea/vomiting, slow heartbeat, severe drowsiness, paralysis. When papaya is taken in large doses, it may infrequently cause serious irritation and ulcers in the esophagus.

Why go through all that, not to mention suffer the abject misery of measles itself, when the vaccine is safe and reasonably effective?

To campaign against a vaccination programme, while the country's children are dying, is tantamount to murder.

Last year two children died in Samoa because the vaccine was wrongly mixed, this year 55 children are dead because of anti-vaxx activity, and more are seriously ill in hospital.

In addition I wonder how many more children survived but will have life-long damage. (Over 100 would be my guess).


ps.our tv news last night after reporting on Samoa mentioned that in Europe that Ukraine and France have also had a bad measles season, although I noticed that neither were as deadly as the Samoa outbreak.

If measles has followed its usual pattern 110 profoundly deaf and many more "hard of hearing", 110 blind enough to be registered fully blind and many more "sight impaired". Oh and another 55 dying later from encephalitis which will emerge in the following year and an unknown number - probably higher - dying from another disease entirely, but because measles wrecked their immune system

Many diseases come back to haunt you later. My Hubby's second ex wife had polio when very little. She escaped mostly unscathed. But - in her 60s it returned to wreek more damage. I had never heard of late effect polio, that too is horriffic. Many diseases do more damage much later. We knew about the link between chicken pox and shingles for a long time. But it now appears that many other illnesses play the same nasty peek-a-boo game.

Maybe it is dying down a bit. Daily new cases are half of what they were a week ago, but the death toll keeps going up.

I was talking to 3 doctor friends I have last week and they were telling me that the reason the mortality level is so high in Samoa is that there is absolutely no in built immunity in the population there unlike in Western cultures. Most of us have an ancestor who had measles and survived it and therefore, within us, lies a small amount of immunity. Samoa has never been exposed to measles before.
Those who got it wrong, for whatever reason, may feel defensive and retrench into a position that doesn’t accord with the facts.

As the target for vaccination is reached (95% cover) and the number of new cases starts to drop... Samoa lifts state of emergency

Quote Samoa has lifted a six-week state of emergency, which was put in place amid a measles epidemic that killed 81 people and infected more than 5,600.

Just 200,000 people live on the South Pacific island nation, and vaccination rates are far lower than in neighbouring countries.

Most of those killed in the outbreak were babies and young children.

Infection rates slowed earlier this month after a vaccination drive pushed immunisation rates towards 95%.

According to aid agencies, this is the level required in order to have "herd immunity" against the disease.


The latest Government report
Quote Latest update: 5,667 measles cases have been reported since the outbreak with 12 recorded in the last 24 hrs.

To date, 81 measles related deaths have been recorded.

VACCINATION UPDATE:As of 28 Dec, 95% of population have been vaccinated.

This is rapidly running down. Instead of 50+ cases a day, there have been just 30 in the last week or so (and two more deaths). The daily twitter updates have also stopped. So the statistics below are close to what the final figures will be.

Of concern is that 5% of those hospitalized have either died or are still in hospital (99 cases), which is a high percentage for measles. According my calculations from the figures given below there should be around 16 patients still in hospital.


Quote UPDATE ON THE MEASLES OUTBREAK

Press Release; HEALTH EMERGENCY OPERATION CENTRE, (HEOC)

January 6th, 2020; The Ministry of Health confirms a cumulative total of 5,697 measles cases have been reported to the Disease Surveillance Team, since the outbreak started.

Currently the outbreak is declining with 30 new cases reported between December 29, 2019 and January 5, 2020.

As of 8.30am January 5th 2020, there were 16 measles currently admitted at the Tupua Tamasese Meaole Hospital (TTMH) including 4 critically ill children in ICU/HDU.

The total number of measles cases admitted to all hospitals recorded for the outbreak to date is 1,860. Of that, 1,761 (95%) patients have recovered and been discharged.

To date, 83 measles related deaths have been recorded. There were 2 fatalities (one infant and one adult) between December 29, 2019 and January 5, 2020.

Measles Outbreak in the Pacific - Situation Report No 9, December 31, 2019

Event highlights

β€’ As of 30 December, four Pacific Islands Countries and Areas (PICs) have announced measles outbreaks: American Samoa, Fiji, Samoa and Tonga. The number of cases is decreasing in Samoa, and the situation has stabilised in American Samoa, Fiji, and Tonga.

β€’ Kiribati has two laboratory confirmed cases of measles, no epidemiological link has been identified during investigation.

β€’ A suspected measles case in Niue has been found to be laboratory negative; i.e. no case of measles.

β€’ The situation in other PICs is being closely monitored with on-site and remote preparedness support from WHO, UNICEF and other partners.

Summary by PIC

β€’ American Samoa: No new measles case reported since the last report.

β€’ Fiji: one new case is under investigation to assess links to the current outbreak.

β€’ Kiribati: two laboratory confirmed cases, but no epidemiological link identified. One suspected case under further investigation.

β€’ Niue: one suspected measles case in Niue has been verified as negative.

β€’ Samoa: situation is stabilizing, with a decrease in the numbers of reported cases. The Measles State of Emergency ended on 28 December 2019.

β€’ Tonga: outbreak is slowing, with fewer numbers reported, mostly from the main island of Tongatapu.

Pacific Regional:
β–ͺ WHO will begin rolling out a collaborative methodology to work alongside Pacific Ministries of Health to identify and prioritise key measles preparedness and response actions for the next 3-6 months.
β–ͺ Availability of vaccines is currently monitored by WHO and UNICEF (Table 3) and contingency plan is in place in case of additional demand.

Other emergency activities in the region

β€’ Tropical Cyclone Sarai has affected Fiji and Tonga.

β€’ In Fiji, over 2,000 people were evacuated in 48 evacuation centres.

β€’ The Fiji Ministry of Health and Medical services has put measles surveillance in place and offered vaccination.

β€’ Other coinciding events:
β–ͺ There are ongoing dengue outbreaks in the Kosrae and Yap States in FSM and Republic of the Marshall Islands (RMI).
β–ͺ Influenza outbreak reported in Republic of the Marshal Islands
β–ͺ Acute Diarrhoea and vomiting (ADV)/Rotavirus RDT-positive cases reported in Kiribati. One death due to ADV has been reported and is under investigation.
