DJ, With most of the testing/reporting on not only CoViD but also M-pox (and H5N1 ?) gone-"the news" on the ongoing healthcrisis is "under control". I will try to find info (here for may 2023) on the ongoing healthcrisis. 

One question to find out is if CoViD is now resulting more in chronic (long term) disease than deaths. 

As seen before infection on a large scale in some regions may offer protection against some new variants (either imported or developing in the same region).

One of the things we've learned in the past couple of decades about influenza is that humans - much like birds and pigs - can be co-infected with 2 or more flu viruses simultaneously. Up until 2009, human co-infection with influenza A had only rarely been reported. 


Dual infections had been noted, but usually with an `A’ and a `B’ virus. 

In 2008, we saw a report of an Indonesian teenager who tested positive both H5N1 and the seasonal flu strain H3N2 (see CIDRAP report Avian, human flu coinfection reported in Indonesian teen), demonstrating that co-infection with Influenza A was possible.

 But the big breakthrough came in 2009 when the pandemic H1N1 virus emerged and co-circulated for a time with the old seasonal H1N1 virus, allowing scientists in New Zealand to isolate and document 11 cases of Influenza A co-infection (see EID Journal: Co-Infection By Influenza Strains for the full story).

Since then we've seen a growing number of documented human co-infections, and more importantly, the detection of novel flu reassortants originating from them. 

DJ...not only pigs but also humans can be coinfected with different flu-virusses in the same time...

Luckily, most reassortant viruses aren't biologically `fit' enough to compete with more established strains, and quickly fade away, often without ever being noticed.  If creating a pandemic virus were easy, we'd be hip deep in them all the time. 

But with record levels of avian H5N1 being reported globally, the opportunities for it to meet up with a better adapted seasonal virus in a human host are increasing. And that raises the likelihood that an H5N1/H1N1 or H5N1/H3N2 reassortant could emerge. 

After three years of the COVID pandemic, and round-after-round of COVID shots, many people are weary of getting vaccines, including for seasonal flu. While understandable, not vaccinating against seasonal flu provides additional opportunities for H5N1 (or H5N6, H3N8, H9N2, etc.) to reassort in a human host.

-Reduce (global/air) travel...we are transporting all kinds of diseases all over the planet 24/7...Why ? Profits ? At the end pandemics may kill us all...not worth the "profits/economy"!

-Masks have been used hundreds of years to limit infection risks...certainly in combination with eye-protection they may limit infection risks...for lots of diseases...

-Ventilation lowers "disease levels" in buildings (hospitals, schools, offices) ...also low cost, very simple...

-UV-C lights have been used in the past to limit diseases...

-Work/study from home more often -may also limit traffic jams, air polution...

-Improving air quality also limits lung (etc) damage...les fossil fuel burning=better air quality !

-Improving your health, ride a bike ( a bicycle...not a motor-bike !), walk more...

-Stress reduction also helps...A vaccation nearby can make you enjoy the country/place you live in...

So...lots of things YOU !!! can do !

Vaccines have become a conflict-zone...If you do not want CoViD-vaccines make up your mind on flu-vaccines (non mRNA)...Vaccines did save millions of lives !

Where I have added my view on this WHO decision too... Esentially, regardless of whether it is considered as a pandemic, Covid remains a significant problem across the world, especially for Long Covid & for the clinically extremely vulnerable.

DJ...in countries we keep poor there may be NO TB-testing...most TB-cases worldwide are missed ! Previously the ECDC had been publishing these highly detailed surveillance reports every 3 months (see here and here), but today - less than 60 days since the last release - they've published a summary for the months of March and April. 

As the chart above illustrates - prior to 2019 - avian epizootics were rare, and sporadic events in Europe.  Over the past 3 years, however, the virus has become endemic, persisting (albeit at lower levels) throughout the summer.

While H5N1 is the dominant subtype, there dozens of genotypes circulating in Europe, with even more globally. Some of these genotypes are likely more dangerous, or better adapted to mammals, than others. 

Over time, the virus has also expanded its host range, both in mammals and wild bird species (see chart below), moving from primarily affecting waterfowl (in blue) to heavily impacting shorebirds (in green).

While H5N1 is current at the top of our hit parade, it isn't the only pandemic threat out there.  

The CDC actively follows 23 subtypes of Zoonotic Influenza on their IRAT List, and as we discussed yesterday in China: Emergence of a Novel Reassortant H3N6 Canine Influenza Virus, new subtypes with pandemic potential continue to emerge. 

Today's report also addresses recent human infections by H3N8, H5N6, and H9N2 as well. 

A reminder that we live in a threat-rich environment, and we need to prepare for a variety of scenarios going forward.

The HPAI H5N1 virus has been with us since the mid-1990s, but the virus of today is a very distant relative of that original virus.

HPAI viruses - which were common in poultry (aka `fowl plague') - had never been detected in wild or migratory birds prior to 1996. That year a previously unknown H5N1 virus (clade 0) was detected in a domestic goose in Guangdong Province, China (A/Goose/Guangdong/1/1996 (GsGD)).

The following year H5N1 appeared in Hong Kong's chickens, and sparked the first human epidemic, hospitalizing 18 and killing six (see Outbreak of avian influenza A(H5N1) virus infection in Hong Kong in 1997). 

The bold decision to cull every chicken in Hong Kong, and the immediate isolation of cases, were credited with stopping that first outbreak. 

The H5 virus appeared to go to ground for the next five years, but re-emerged again in early 2003 - albeit as a new clade - in Hong Kong when two members of a family fell ill (one fatally) after returning from Fujian Province on the Mainland. A third relative died (untested) on the mainland.

Within a year, new variants of H5N1 virus caused outbreaks in poultry, wild birds, hundreds of humans, and dozens of captive animals from Beijing and Korea to Thailand and Vietnam. 

Two years later - after a new clade emerged at Qinghai Lake (China) - H5N1 spread out of Asia and moved into Europe, Africa, and the Middle East.

And as H5N1 expanded geographically, it expanded genetically as well (see chart below), forming numerous new subtypes (e.g. H5N6,H5N8, H5N5, etc.), new clades, subclades, and genotypes

Many of these variants were less `fitthan their competition, and died out along the way, but others thrived and survived, and became the dominant clade in different parts of the world.

Over the years we've seen several sudden expansions of the H5 virus (2005, 2009, 2015-16, and most recently 2020-21).  Each one occurred after a major evolutionary change to the virus (new clade or new reassortment).  


While today's H5N1 virus is thankfully not spilling over into humans at the rate same we were seeing in the mid 2000s (see chart below), it is far better suited for long distance carriage by migratory and wild birds, has spread globally at a phenomenal rate, and continues to experiment with new mammalian hosts.  While the future course and impact of HPAI H5N1 is unknowable - and the threat may recede as it has in the past - these recent developments are red flags we can't afford to ignore.

A 61-year-old man, who lives in Vitória, in Espírito Santo, may have been the first infected in Brazil with the H5N1 virus, which causes avian flu. The suspicion was notified on Wednesday (17/5), informs the Ministry of Health (MS).

Five weeks ago, in in U.S. Fish & Wildlife Service Statement: HPAI Confirmed As Cause Of Death For 3 California Condors, we looked at a preliminary report on the deaths of 3 Condors - and the investigation into additional deaths - in northern Arizona due to HPAI H5N1.

Since then we've seen three updates (hereherehere ) where those numbers continued to climb. As scavenger birds, Condors feed almost exclusively on carrion, which increasingly runs the risk of being infected with HPAI H5 (see USDA Mammalian HPAI Infection List). 

The critically endangered California Condor nearly became extinct in the early 1980s, but has pulled by from the brink due to conservation efforts by the US Fish & Wildlife Service, the National Park Service, and breeding programs at San Diego's Wild Animal Park and the Los Angeles Zoo.


Today there are roughly 500 birds in captivity or in the wild, and H5 avian flu poses a genuine risk to the survival of the species.

One year since its biggest ever mpox (monkeypox) outbreak, the WHO European Region – covering 53 countries across Europe and central Asia – is celebrating success in controlling the spread of the disease as the number of cases continues to decline. In the initial months of the outbreak, the European Region had the vast majority of cases globally. But strategic collaboration between health authorities and the most impacted populations – including timely risk communications and community engagement, along with a focused mpox vaccination campaign in some of the hardest hit countries – helped bring numbers down rapidly to practically no cases being reported at all, in a relatively short time.

Now WHO/Europe is calling for vigilance amid concerns that spring and summertime could see mpox cases flaring up once more. Pre-empting this, and coinciding with the International Day Against Homophobia, Biphobia and Transphobia, WHO/Europe is launching a new mpox campaign today, to remind people that even though it is no longer defined as a public health emergency of international concern, mpox has not gone away; anyone can be infected; and that we have, and must use, the tools to further control and eventually eliminate it.

While overwhelmingly a `humanized' pathogen, SARS-CoV-2 has also found a plethora of other suitable hosts, including mink, deer, dogs, cats, and even rodents (see Nature: Comparative Susceptibility of SARS-CoV-2, SARS-CoV, and MERS-CoV Across Mammals).

This co-circulation in non-human hosts could provide COVID with new opportunities to adapt and evolve, and potentially spill back into humans at a later date.  

We saw an example of this kind of parallel evolution a month ago in Eurosurveillance: Cryptic SARS-CoV-2 Lineage Identified on Two Mink Farms In Poland, when we looked at the detection of two closely related COVID variants that turned up - 3 months apart - at two mink farms in Poland.

Both closely matched a (pre-Omicron) variant  (B.1.1.307) not seen in humans in nearly two years. At the same time both showed clear signs of continued evolution (at least 40 nt changes) since its last appearance in humans. 

Between vaccines and acquired immunity, older variants have a limited shelf-life in humans and are supplanted by new variants fairly quickly. But they may be maintained in other animal populations, where they continue to evolve and adapt (see PNAS: White-Tailed Deer as a Wildlife Reservoir for Nearly Extinct SARS-CoV-2 Variants).

Over time, we've seen SARS-CoV-2 adapt to new species, including those that did not appear susceptible to the first wave of the pandemic (see PrePrint: The B1.351 and P.1 Variants Extend SARS-CoV-2 Host Range to Mice).

Early assurances that pigs, cattle, poultry, and other common livestock were not susceptible to COVID are constantly being reevaluated (see EID Journal: SARS-CoV-2 Antibodies Detected In Cattle Suggest Limited Spillover - Germany).


All of which brings us to a remarkable study, published this week in the journal Viruses, which describes the finding of SARS-COV-2 in large number of farmed, or peridomestic, animals in Southern Nigeria. 

In fact the number is so high, my first inclination was to suspect cross-contamination. While the authors address this concern, they believe their methods were sound. 

Animals tested came primarily from in and around small family farms in rural Nigeria, where the human-animal interface is much closer than with large commercial farms. Surprisingly, they found evidence of SARS-CoV-2 infection in poultry, cattle, goats, and even lizards

SA Health said eight COVID-19 deaths had been notified in the past week, in people aged in their 70s to 90s. 

There are currently 140 people in hospital with COVID, a figure Professor Esterman said "wouldn't be causing SA Health too much concern".

"If it got to double that number then they would start getting really worried," he said.

A new case of the latest Covid-19 sub-variant has been reported in Thailand. The XBB.1.16.1.1 or “FU.1” is said to be 50% more transmissible than XBB.1.16, which is one of the dominant Omicron strains in the country.

The Center for Medical Genomics of Ramathibodi Hospital has posted on its Facebook page about the continuous mutation of the Omicron variant around the world. Currently, the dominant Omicron sub-variants are XBB.1.5, XBB.1.16, and XBB.1.9.1, with the latter two being the dominant strains around the world including in Thailand.
According to reports, XBB.1.16 was found in 9,003 Covid-19 cases globally, while 139 cases were found in Thailand. XBB.1.16.1, was discovered in 2,714 cases worldwide and 26 cases in the country. XBB.1.16.2 was detected in 666 cases globally and 25 domestically, while XBB.1.16.3 was found in 175 cases around the world while five cases were found in Thailand.

Bulgaria: 120 new Covid cases in the last 24 hours. 300 people with a diagnosis of COVID-19 are in hospital, of which 29 are in intensive care units. There are 32 new hospital admissions, and 3 deaths were reported.

The Ministry of Health was notified, this Saturday morning (20), about the negative result for avian flu in all employees of Parque da Fazendinha, in Espírito Santo, where a sick bird was found. Of the 33 employees, only one was under observation as a suspected case of the disease, as he had flu-like symptoms during the week. The result released today informs that the suspected case had a negative laboratory result for all targets tested – that is, it was discarded for Avian Influenza (H5N1). Of the other 32 samples, 30 were negative for all targets tested and 2 tested positive for viruses that were already in circulation (Influenza A and Influenza B).

For the past two decades the World Organization for Animal Health (WOAH, formerly the OIE) has stated that the `preferred' method for dealing with HPAI H5 or H7 in poultry is stamping out (culling), and that vaccines should be viewed as a `temporary' solution at best.

While billed as the first `unrestricted' Hajj since the start of COVID, there are still minimum vaccine requirements, and pilgrims must provide a negative COVID PCR test conducted no more than 72 hours before arrival in the Kingdom.  

Reports on the other coronavirus of concern - MERS-CoV - have plummeted since 2019, although the virus remains endemic in camels, and there are doubts over the thoroughness of surveillance and reporting by many Middle Eastern countries.

-International situation now has tens-of-millions of people on the move to places with better perspectives/future

...of course Mpox is NOT over...increase warnings summer may see new spread/cases. But in the "media" Mpox no longer is "news"...Pandemic fatigue has most of the public ignoring risks...Politics actively push for denial of any crisis...(and promote war)...

DJ, In Brazil now 102 people under investigation-92 of them test negative, so 10 cases wait for further outcomes...Since Brazil is part of BRICS/BRIICSS+, Brazil, Argentina dumping the US$ for international trade...is the US going for bio-war against Brazil ? The US keeps making claims China was to blame for a "lab-leak" start of CoViD...without any realistic proof-the opposite -gradual development- seems so much more likely...

H5N1 is "on a global tour" now also reaching Brazil...(first time ?) ...It is also winter, CoViD is not gone...

DJ...my basic problem with (non-sterilizing) vaccines is they may not be able to stop a healthcrisis on the long run...People still get infected-the vaccine is meant to stop major disease. Sterilizing vaccines stop infections...

To get out of this global health crisis-with very likely over 100 million people worldwide dealing with long term CoViD related illnesses-we need to change the way we live...

-Limit travel (=spreading disease)

In the 25 to 44 age group, the increase was 31 percent. Young people under 25 did not seek help more often.


“It is a development that worries us,” said Michel Dukers, professor by special appointment of Crisis, Safety, and Health in Groningen and the research leader on behalf of the RIVM and Nivel. “We still don’t know much about the long-term effects, but the picture is now emerging that the pandemic can lead to faster aging on a significant scale.”

Among people over the age of 75, 18 percent more reported cognitive problems to their GP than in 2019. “The increase there is probably less strong because people of this age already had cognitive problems more often than other age groups before the pandemic,” said Duckers.


The researchers haven’t formally proven that the increased cognitive problems are due to the pandemic, but they believe it fits with the growing knowledge about the consequences of the coronavirus. How the virus may cause these problems is not yet fully understood.

One explanation may be that the lockdown measures against Covid-19 led to a faster decline in people with early memory and concentration problems. Researchers at the Alzheimer Center expected at the start of the pandemic that the social restrictions might lead to an increasing group of people with mild memory and concentration problems.

According to Dukers, the virus itself may also be behind the problems. Some people have long-term complaints after a Covid-19 infection, and several studies have shown that memory and concentration problems are common in Long Covid

More higher-income people needed financial aid last year

The Dutch Association of Public Credit (NVVK) also reported that people with higher incomes sought financial help more often than unusual last year. The sector organization based this on data from municipal departments for financial aid and from foundations and private organizations.
