Here's a report from the UK's Daily Mail of a mysterious, fast acting, pneumonia in Argentina.

DJ-I mentioned it yesterday-a twitter/Spanish article link. TMN claims that Brazilian experts think it may be a recombination-CoViD-variant. The Argentine Tucumán region -now coming out of winter-has high HIV, did see lots of other diseases-also in animals.

CoViD may have been widespread but lack of testing/sequencing may have resulted in a test evading CoViD recombination variant.

DJ-So it is still a bit open...if it is a "bad CoViD variant" the ongoing pandemic will only get worse...the variant very likely already spreading....

They analyze the samples of the cases of pneumonia of unknown cause in Tucumán
listen to this note

The National Administration of Laboratories and Health Institutes (ANLIS) “Dr. Carlos Malbrán” received the samples from Tucumán to carry out a diagnostic extension of the six cases of pneumonia of unknown origin.

The registered cases correspond to five health workers and a patient from a private sanatorium who started symptoms between August 18 and 22.

In addition, the health authorities are working to carry out the investigation of the outbreak, which includes the follow-up of the contacts of the cases and specific control actions in the health center where the outbreak arose.

The registered cases correspond to five health workers and a patient from the intensive care unit of a private sanatorium in San Miguel de Tucumán who started symptoms between the 18th and 22nd of this month. The six cases presented bilateral pneumonia, two of them died, three are hospitalized and the remaining one is in good health in home isolation. Since August 22, no new cases have been detected.

DJ, One may hope results will be in in a short time-so spread may be limited. So far there are no Non Pharma Intervention steps...countries closing borders for travel from Argentina.

DJ latest information may indicate spread outside the healthcare facility. So legionella is not very likely...

Samples have been sent to the prestigious Malbran Institute in Buenos Aires for further analysis. Contacts of the first patients are being tracked and the healthcare centre has been isolated.

That is a long time to have no new cases.  I was worried more until I read that so now don't feel as worried.  Are they the type of country that would hide information to avoid loss of income from tourists?  What are your thoughts PT?

I follow https://twitter.com/HenrikAborym or https://twitter.com/HenrikAborym who is following the Argentina-Tucumán story...

https://msptucuman.gov.ar/situacion-actual-del-brote-de-neumonia-bilateral-en-la-provincia/ or https://msptucuman.gov.ar/situacion-actual-del-brote-de-neumonia-bilateral-en-la-provincia/

During the morning of today, the head of the health portfolio, Dr. Luis Medina Ruiz, together with the Medical Executive Secretary, Dr. Miguel Ferre Contreras, gave a press conference from Government House to update the population on this problem.

"We want to announce that, regarding the cases of bilateral pneumonia with unknown etiology and under investigation, we continue with nine patients, there are no more people added to the list", assured the minister.

And he continued: ¨The novelty is that we have a patient who, yesterday, at night, was discharged, hospitalized and continues to be observed at home due to her clinical condition. At this time we have three patients who are still hospitalized, of which two are on mechanical ventilation and one on non-invasive ventilation; All three in serious condition. The other three patients are in an outpatient condition with less serious follow-up and, unfortunately, three have died¨

DJ, So if I get that correct; 9 cases, 3 died, 3 at ICU and 3 send home with improving symptoms...

¨We received information that the studies that are being done so far are negative for bacteria and viruses that are being investigated. In any case, there are studies that require cultures, so they require a little more time¨, explained the official. Meanwhile, he added that when the results are negative, another study is sought that is more sensitive and complex, which sometimes takes longer to perform.

"Surely in the course of the day we will have other results, the idea is to continue working on the investigation of the cause and we will continue looking for epidemiological links," said the official.

DJ...not clear what it is/was...if it would be a sort of flu I think they may have found it ?

On the other hand, the minister confirmed that at the moment there are nineteen hospitalized patients in the private clinic, some in intensive care and others in common rooms: "We from the Ministry of Public Health have formed teams that are going to support and evaluate the epidemic situation of the patients. They are working on the control of internees, they are analyzing the epidemiological links and they are analyzing everything related to environmental health; We do not leave any edge uncovered.

He also added that, so far, they have not found any pathogens in the water and the results of chemical tests are pending.

Regarding the time in which new patients can present some symptomatology, the head of the health portfolio said that, whenever it is a viral or bacterial disease, the process time can last between seven and fourteen days, a time that Health it will take to wait if any new patients show up.

DJ, MPX incubationtime up to 3 weeks...if it is "something else" (form of allergy, poisening...it can take much more time...)

¨The fact that there are no more infected encourages us to think that it is an infectious agent that does not have a great power of contagion. Usually all viruses, bacteria and fungi have the ability to spread through a medium; This, apparently, is a case that does not produce significant contagion, or the other possibility is that it is a contagious germ with a longer incubation period, he extended.

For this reason, the minister asserted: ¨We are constantly having meetings at the national level and we have the full support of institutions of national and international prestige to reach the result of the etiology. It is essential that the information on this topic is carried out in an official and formal manner. The greatest risk occurs when there is too much misinformation and it creates panic in society.

Finally, Medina Ruiz assured that the health control carried out by Siprosa at the level of the personnel, as well as the health of the place, will be reinforced in this situation.

DJ..The private healthcare facility has 19 patients...however "the unknown disease" most did cause symptoms in healthcare workers ????

Argentinian vice president Cristina Fernández de Kirchner survived an astonishingly close encounter with death on Thursday night, as a would-be assassin pointed a loaded pistol at her head and seemingly pulled the trigger with no effect.

The gun was loaded with five rounds, according to President Alberto Fernández. "Cristina remains alive because, for a reason not yet technically confirmed, the gun, which contained five bullets, did not fire," he said in a nationwide television broadcast, describing the attack as "the most serious incident since we recovered democracy” in 1983.

Fernández de Kirchner blinked and ducked down as first the crowd and then her security detail reacted. The assailant was quickly apprehended as he stood amongst her backers. Though his name has not yet been released, authorities say he is a 35-year old of Brazilian origin.

Fernández de Kirchner is embroiled in controversy:

"The attack came as the vice president is facing a trial for alleged acts of corruption during her 2007-2015 presidency — charges that she vehemently denies and that have led her supporters to surround her home in the upscale Recoleta neighborhood of Argentina’s capital," reports the Associated Press.

She stands accused of defrauding the government and corruptly awarding public works projects to her political base in Patagonia. Prosecutors are seeking a 12-year sentence and a lifetime ban from politics. As Senate president, however, she enjoys immunity, which could end with her losing a late-2023 election or if the Supreme Court ratifies her sentence.

Here's an update from the British press. I'm with Roni on this one, it's worrying but the longer we go without new infections the better the outlook seems to be but I do think this is one we have to keep an eye on.

“The Argentine Ministry of Health reports that a new [tenth] patient linked to the conglomerate of cases of pneumonia of unknown origin has been detected.”

Case number 10 is an 81-year-old male patient with comorbidities hospitalized in serious condition, with ARM, in the public sector.

At this time we have three patients who are still hospitalized, of which two are on mechanical ventilation and one on non-invasive ventilation; All three in serious condition. The other three patients are in an outpatient condition with less serious follow-up and, unfortunately, three have died¨.

At the moment, negative results have been obtained in urinary antigen for Legionella, detection of Antibodies for Coxiella, panel of 12 respiratory viruses, hantavirus, Elisa IgM, real time PCR for histoplasma and microagglutination for leptospirosis. In addition, today genetic material was delivered to the genomic platform for total DNA sequencing using the metagenomic technique.

We had information from the Malbrán Institute and so far all the studies that we had as negative -for Covid, Hantavirus and some strains of Legionella- are also giving negative results there. We continue to carry out the research protocol for blood cultures, sputum cultures, urine cultures and all the viruses and bacteria that we have available in the province.

Press release 2 – 2nd September 2022 – 10 cases, three deaths (in Spanish)

30th August – notification of incident to PAHO

So still unclear what happened here...less and less likely it is a very infectious disease...we would see much higher number of cases then...Still loads of ?????

Legionella suspected:

https://www.lanacion.com.ar/sociedad...s-nid02092022/

(I do note that the rapid tests of Legionella only detect the most common strains and the full culture must be done to confirm some of the others. There was a mystery pneumonia outbreak in Ontario, Canada a few years ago that was initially reported to be ruled out for Legionella because of those tests that was later confirmed by culture. The presence of cases lined to the medical facility itself and not to the activities of patients outside that facility also suggests an environmental exposure like Legionella.

While the article reports that the drinking water tanks were negative for all bacteria, having been sufficiently chlorinated, other water sources such as heaters, air conditioners, or humidifiers/dehumidifiers have been implicated in Legionella outbreaks elsewhere. -

The article also increases the number of patients that have been discharged to three. - alert)

On 5 November 2020, BBC News reported that 12 cases of human infection with the cluster 5 variant had been detected.[28] A week later, an ECDC rapid risk assessment report indicated that 214 mink-related human cases had occurred,[27] however, few of these, if any, are believed to have been additional cases related to the Cluster 5 outbreak.[29] By 20 November, no further human cases of the Cluster 5 strain were being detected despite widespread genetic sequencing which revealed 750 cases related to mink, and it was assessed that the Cluster 5 variant was no longer circulating in humans.[30]

DJ;

https://www.coronaheadsup.com/coronavirus/argentina-two-more-deaths-linked-to-pnuemonia-cluster-in-tucuman/ or https://www.coronaheadsup.com/coronavirus/argentina-two-more-deaths-linked-to-pnuemonia-cluster-in-tucuman/ ;

The Argentinian authorities have announced two more patient deaths from the cluster of pneumonia cases that was announced last week. The cause of the outbreak has been provisionally determined to be legionella bacteria.

We believe that six people in total have now died in the outbreak, but there is no official total count mentioned in the press releases. All of the patients at the Sanatorium where the outbreak first occurred have been moved to a public hospital over the weekend.

-

UPDATE 1 – 5th September 2022 -8 new patients confirmed

The province’s Ministry of Health reported today eight new cases related to the outbreak of legionella in the province .

According to official data, they are two health agents, one patient and five internee caregivers. In this way, the number of those affected rose to 19 since the beginning of the infections , around August 18 last: six deaths , nine patients with outpatient medical follow-up at home and four hospitalized (two with mechanical respiratory assistance).

-

Today, knowing that it is a bacterium that does not always cause bilateral pneumonia, we are including any health personnel, patient or family caregiver who has been in the month of August or has received a surgical intervention in the selection criteria for patients. in this private sanatorium and who has some compatible symptom such as fever or respiratory symptoms, not necessarily bilateral pneumonia, “he reported.

“That is why today we are including at least 8 new patients, of which 1 is a patient, 2 are health personnel and 5 are caregivers of patients who were hospitalized, post-surgical or with neurological problems. Of these 8 patients, there is one who has severe comorbidity, due to a cerebrovascular accident and is also suffering from pneumonia, he is the only one who is serious. We are very sorry for the death of the two patients yesterday, who were on mechanical respiratory assistance for their bilateral pneumonia, and very seriously ill with severe comorbidities. One of them was 81 years old and the other 64, in summary today we have 6 deceased, 9 patients who are at home with outpatient medical follow-up, since the clinic they present is of moderate or mild severity, which allows them to continue in their home and 4 hospitalized patients, two of whom are seriously ill on mechanical ventilation (1 patient is with a cannula and the other with room air without oxygen but hospitalized for severe comorbidities) and we are waiting for the appearance or not of symptoms that allow them to continue in therapy or eventually be discharged from the hospital,” he added.

Tucuman press release 3

and https://www.thailandmedical.news/news/latest-argentinian-mysterious-fatal-pneumonia-outbreak-that-authorities-claim-is-legionella-is-still-spreading-with-22-infections-and-6-deaths-so-far or https://www.thailandmedical.news/news/latest-argentinian-mysterious-fatal-pneumonia-outbreak-that-authorities-claim-is-legionella-is-still-spreading-with-22-infections-and-6-deaths-so-far In short;

-Normal used legionella anti biotics do NOT work

-Spread outside the private hospital (also another health care facility now has 2 doctors in ICU).

DJ-Legionella may take some time for first symptoms, if legionella did also had a lot of time to spread via a watersystem it could show up in more then one location and also over a longer time...If treatments do not work it "could be a new type of legionella"...

Major point is to get the outbreak under control...not what the name is...

https://www.worldometers.info/coronavirus/weekly-trends/ or https://www.worldometers.info/coronavirus/weekly-trends/  (yesterday claimed Argentina did stop reporting cases-may have been an error in a database...)

Argentina cases last 7 days 9,974 the week before 18,663 -47% Brazil cases still +21% ...so is there a new variant/virus spreading in some area's of South America ?

https://www.worldometers.info/coronavirus/country/argentina/ or https://www.worldometers.info/coronavirus/country/argentina/ and https://www.worldometers.info/coronavirus/country/brazil/ or https://www.worldometers.info/coronavirus/country/brazil/ both have had much higher numbers in the past...South America is moving towards winter...

That's interesting that they are provisionally calling it Legionella disease because you can get a positive test result back in 72 hours but it takes over a week to get a confirmed negative culture back from the lab. Legionaires disease is common here because of our climate it is normally caught from compost in bags.

Also interesting that the normal antibiotics used to treat Legionaire's are not working.

Those who got it wrong, for whatever reason, may feel defensive and retrench into a position that doesn’t accord with the facts.

DJ,

https://www.coronaheadsup.com/health/legionnaires-disease/who-legionellosis-outbreak-in-tucuman-argentina/ or https://www.coronaheadsup.com/health/legionnaires-disease/who-legionellosis-outbreak-in-tucuman-argentina/ ; (part)

Legionnaires’ disease, the pneumonic form, has an incubation period of 2 to 10 days (but up to 16 days have been recorded in some outbreaks). It is an important cause of community- and hospital-acquired pneumonia; and although uncommon, Legionnaires may cause outbreaks of public health significance. Initially, symptoms are fever, mild cough, loss of appetite, headache, malaise and lethargy, with some patients also experiencing muscle pain, diarrhoea and confusion. The severity of Legionnaires’ disease ranges from a mild cough to rapidly fatal pneumonia. Untreated Legionnaires’ disease usually worsens during the first week.

Mortality from Legionnaires’ disease depends on the severity of the disease, the use of antibiotic treatment, the setting where Legionella was acquired, and whether the patient has underlying conditions, including immunosuppression. The death rate may be as high as 40–80% in untreated immunosuppressed patients and can be reduced to 5–30% through appropriate case management, depending on the severity of the clinical signs and symptoms. Overall, the death rate is usually between 5–10%.

WHO: Legionellosis – Argentina

-First case was a 70 y/o woman visiting the clinic with healthissues...she was allready ill...

-Cases showing up in several locations (now also in Mendoza-other part of Argentina)

-Symptoms are different, treatment for legionella (antibiotics) not working (as one would expect from a viral infection...)

DJ So the question now is; IF it is legionella why it is acting in this strange way...IF it is NOT legionella what then ? A new CoViD-variant ? Something else ?

The picture I get is that if cases continue to increase the source of illness is not under control...that at least may indicate it could be another disease...as well...Maybe a mix of a local legionella outbreak AND some other diseases/CoViD variants (BA.2.75 on the increase, better in also evading testing...)....

We cannot solve our problems with the same thinking we used when we created them.
