Volunteers needed for bird flu response

Hundreds of volunteers are needed throughout the region to help man vaccination stations in the event of a bird flu epidemic, officials said Thursday.

Deputy Fire Chief Michael Spano, Fitchburg's emergency management director, said that once a vaccine is available, communities will need to quickly call up hundreds of residents to help dole it out.

"We need to bolster the volunteerism, that portion of it that would be on the ground level, and we're just getting going on that now," Spano said.

Officials are preparing for bird flu -- the name for the deadly H5N1 strain of avian flu that has killed more than 100 people, mostly in Asia -- to come to the United States.

States preparing for a possible bird flu outbreak should focus on how to contain the virus as a vaccine will be unavailable for several months, the head of the Centers for Disease Control said Wednesday.

"Out of the starting gate there won't be any vaccine, it'll take some time. So we've got to look at old-fashioned ways of slowing the virus down," Dr. Julie Gerberding told The Associated Press.

The virus is typically spread from direct contact with contaminated birds, but scientists fear the virus will mutate and be passed person-to-person, which could lead to a pandemic.

Fitchburg and Leominster would each have four vaccine distribution facilities, with about 60 people in each one.

Charles Coggins, Leominster' emergency management director, said the state told him last week that the vaccine, once available, needs to be completely distributed in less than 48 hours.

"It requires a huge number of people," Coggins said.

Each town of about 10,000 people is supposed to have about 70 people to run a center, Nashoba Associated Boards of Health Executive Director James Garreffi said.

About half of those people should have some type of medical background, Garreffi said.

NABH works with Ashburnham, Ashby, Ayer, Lancaster, Lunenburg, Shirley and Townsend.

Garreffi said finding the volunteers is a difficult task, but the NABH is working with the individual town boards to get out the word.

"We'd like to get out some of the most up-to-date information about what people can do for themselves and also provide them with some opportunity to volunteer," Garreffi said.

Spano and Garreffi said retired doctors, nurses and dentists, as well as anybody with basic emergency medical training are the sort of people towns and cities need.

Spano said he wants to train a "core group" of volunteers.

"Those people would be able to disseminate the training among a lot of other people," Spano said.

Officials are being advised on how to perform "just-in-time training" which would give volunteers the necessary training the day they are needed.

Perhaps they should add some security people since vaccine will be limited.   Drive through vacinations might be wiser so they don't create a crowd of people, some of whom will be contagious.

Pandemic flu victims face GP surgery ban

DOCTORS could be forced to ban pandemic flu patients from their surgeries under new guidelines issued by the Royal College of General Practitioners (RCGP) and the British Medical Association (BMA).

The first UK-wide advice for GPs on infection control in the event of a major influenza outbreak outlines how larger practices might be expected to arrange dedicated teams to treat suspected cases, with separate “flu routes” to ensure they do not come into contact with other patients.

However, it advises that smaller practices, with only one entrance, might have to close their doors to flu patients and have notices indicating alternative places where they could be treated.

Patients could also be asked to wait in their cars until the time of their appointment to minimise contact in waiting rooms. Strict cleaning procedures will have to be employed with soft furnishings, magazines, books and toys being removed .

Doctors are also being urged to consider stockpiling supplies such as masks, tissues and aprons in their surgeries in preparation for a potential flu pandemic.

Earlier this year, the Sunday Herald revealed GPs’ concerns that practices could be inundated by patients in the event of an influenza pandemic and collapse under the demand. In a motion to the BMA’s annual conference of Scottish local medical committees in April, Tayside GP Dr Andrew Buist called on ministers to clarify their pandemic plans as a “matter of urgency”.

Chairwoman of the RCGP in Scotland Dr Mairi Scott said the guidance was the first major piece of detailed advice to start doctors thinking about what preparations they might need to undertake. However, she added that, though the advice was useful, it was not known what form of pandemic GPs might face. “I think it will reassure them [GPs], but who knows what is going to happen ,” she said. “GPs are good at coping with the unpredictable – that’s what it is like in primary care general practice as the first point of contact. Anything can walk through the door.”

The guidelines also contain a detailed checklist of additional supplies that GPs might require to minimise the spread of flu, such as disposable gloves, alcohol wipes, liquid soap and tissues.

While it states that some reserves might be held by health boards or at a national level, it adds: “In the absence of further guidance, practices could consider stockpiling at least 50% of their estimated total pandemic needs.”

The advice has been issued amid fears that the deadly strain of avian flu, H5N1, responsible for more than 100 human deaths worldwide so far, will trigger a global outbreak. In April, a dead swan washed up at Cellardyke in Fife was the first bird in the UK found to be infected with H5N1, although no new cases have been discovered since.

Health minister Andy Kerr acknowledged that effective planning for a flu pandemic required the involvement of primary care providers, such as GPs and local pharmacies. He said: “That is why NHS boards are taking full account of primary care needs in their local planning, and why the Executive has not only been talking to primary care planners at board level, but also talking directly to the Scottish GPs committee.”

Watch some biographies of historical events. One not so long ago. On 9/11 we saw firemen, and policment run into death to save others. We saw construction workers working round the clock without respirators. We saw nurses and Dr.s come in from all over the country.We saw common folk on flight 93 take that plane down in Pennsylvania to save the capitol. No one knew on that day, what was comming. We all rose to the occasion. Dont be too fast to rule out the heroism that has been around since day one. Think about five hundred rangers that died to take the beach on d day. They knew they were a human shield. They saw their friends die right before them and they kept on comming. They did not turn and run and hide. My money is on us as a people. We may be down but not out. I beieve we will all step up to the plate.

yeah it sounds good but we are not dealing with muslim cranks with box cutters
 
This is pandemic influenza and it will be spread by people moving around so the more people who stay at home and quarantine themselves the better
 
there will be no vaccines and fluffing around pretending to be a do gooder will only risk your life and the lives of your families and friends
 
This is a major disease for which there is no vaccine and if you want to volunteer for something go to the hospital and body disposal programme because thats where the hands will be needed.
 
I would but my back is shot s I will cut myself off till it blows over
 
H5N1 is like nothing humans have met before and heroics will put lives at risk

I agree kilt. My hopes rest with herd immunity, which is inevitable, and mutation to a milder form. Neither of these can be achieved until the majority of humans have been infected and gained some form of immunity.

The duration of the pandemic is likely to depend on human nature and political strategy rather than the virus itself. In effect the only real defence against this virus is for most other people to have already been infected.

It is in my personal interests that most people have faith in their Gods, their governments and pure chance. I need people to have faith in humanity and medical science. I need people to detest mathematics and be bored senseless with politics. I need people to believe in luck.

I did agree but we crossed in the post.

I don't really want the majority to isolate themselves. It won't help them, it won't help me and it will simply cause a repeat of what happened in Australia in 1918.

In short, whether the majority hides for two weeks or two years, the majority still need to come into contact with the virus before herd immunity can emerge.

some in 1918 had no immunity at all and 100% of them died - the last one alive buried the others and then lay on the ground and died
 
That virus had 3% death rate yet still wiped out 100% of some native communities.
 
we have no idea what will come with this virus

Me too, but the duration of my isolation is totally dependant upon how long it takes for the majority to develop herd immunity and the virus to mutate into a less lethal form.

By an indirect and circuitous route the entire isolation theory relies upon most people ignoring it. I'm not a virologist. Maybe the experts take a different view?

Not that this matters much. Its what the political leaders believe that really matters. Sorry to bring politics into it again.

These vaccination sites are known as PODs (Points Of Dispensing). A typical throughput design goal is about 20,000 persons per day. Staffing is a BIG problem, especially if many people are already ill with the disease. Drive-through PODs have already been tried for seasonal flu vaccination, and were successful, so this method could well be tried for avian flu vaccination.

Treating everytone in 48 hours requires a larger staff at each POD, but has the advantage of vaccination immunity forming in a large population within the incubation time of the virus. Thus, the more volunteers are available, the more quickly immunity is built up in the community. It is in YOUR self-interest to volunteer.

What's that skippy? No what? Oh I see. No vaccine. Listen skippy, don't bother me with details. Just give em some of that new stuff. You know, that stuff that hasn't been tested yet. Trust me. It'll be O.K.

What's that skippy? Me? Well I'd like to skippy but no, I won't actually be coming down there myself. I'll just stay here and look after the preps.

You'll be back later? Well actually I don't think that dog's going to run skippy. You see, you might be immune, but you can still carry the virus. I know its just another one of those troublesome details but I've always been picky about how I die. Good luck skippy.

Linn County Ready For An Outbreak

CEDAR RAPIDS, IA - Volunteers could help fight flu outbreaks, bio terror attacks and other public health emergencies.

Sen. Tom Harkin (D-Iowa) and some of his colleagues introduced a plan for more local, volunteer medical response teams.

Linn County already has a Medical Reserve Corps, similar to what senators are proposing for the nation. Together with the Public Health Department and Emergency Management, local medical providers say they have an adequate amount of trained volunteers ready for almost any disaster.

Officials say, if faced with threats like avian or pandemic flu, their system should work - from quarantined areas to widespread vaccinations.

  The best test for the county was this year's massive mumps outbreak. Officials used the vaccination clinics as a drill for what might lie ahead.

They say, if another kind of outbreak hits Linn County, their results show they're more than capable of stopping its spread.

Here in Michigan  the hospitals are putting their staff on notice. The dept. heads were given notce at a meeting and they in turn talked to employees under them.  DH's boss was visibly upset when he returned and told his group (Lab) that they would have to start thinking about whether or not they will show up for work.  If they chose to be there, then would have to stay on the job.........    DH  has decided to stay.....I told him that I  I respect his choice and will not insist  he stay home as long as he is doing it for the right reason and is not making his choice  out of  obligation  only.
This way I know that if he doesn't  survive this, I will at least know he did it for the right cause..............Cry.   I can't believe I'm having to say all of this...      I hope this all turns to be  just a really bad dream....

When Medical Reserve Corps are recruited for, they talk about vaccinations, because a) local MRCs are called up every year for things like vaccinations, so that is a way of training people, and that they can understand and b) in a pandemic before the vaccine, MRCs would find they have other ways in which they can help.  They helped with Katrina and refugees from Katrina all over the US.
Mary R, bravo, it is good to hear from those of my inclination rather than being ridiculed as stupid for not maximizing my chance of survival.

  ya know, on the flipside of this we need to take into account the mentality that had people in the other planes quietly waiting for help that didn't come.  those people died because they were tought to be nice and let the pro's come and take care of you.  you should do a little research on the airlines that NEVER get hi jacked, el al, japanese airliners [this as more of a perception thing] royal jordanian air, and the old aeroflot.  the only way to be safe and secure is to secure yourselves.  nothing else will work. so the next time someone threatens you with a boxcutter, take your traytable, bash his darn brains out, and stick the boxcutter were it belongs.

I think that to say 9/11 was about box cutters is sad and insulting. The people on flight 93 knew that three other planes went down and they acted.In addition to that, no one knew where the next attack could come from. Furthermore, the rangers on D day, saw their friends drop before them dead and moved on.  Every hour of everyday there are people who put themselves at risk to help others. Live organ doners for kidneys. All these people could die. No one ever suggested fluttering around volunteering, you can leave a plate of food on a elderly persons door, ring the door bell and move back. You will be a hero to that person.  People who suggest that others who do heroric things are fluttering about or being do gooders, are usually cowards and ashamed of themselves.  Cudos to Mary R and to Dlugose, cudos to Albert who has already stepped up.
The soldiers in Iraq who go out on patrol, knowning their friends were killed by a improvised explosive divise are also looking at death or dismemberment not a comforting thought at 20 to go through life without arms or legs, yet they do so for operation Iraqi Freedom, imagine what they may do for Operation America lives or dies. There are examples all the time. I have seen middle schoolers in youth groups go into hospital rooms to hand out Christmas gifts to patients in quarentine. So do what you want and to your own level of comfort. Just dont insult those who have given all, or who would chose different then you.

  i don't believe that anyone said that 911 was about boxcutters, it was just part of an example.  as far as cowards go, i spent 8 years in the us army, and many years since then working as a police officer in exclusively in violent housing projects.  i come from a family with a long tradition of military service here and in ireland/scotland before here.  my brother was a recon marine an my sister was an airforce officer.  my uncles on my mothers side were the first 8 men to volunteer for military service in wash pa on dec 8, 1941.  another uncle [paternal] spent 18 months in a german pow camp, all of my uncles were decorated to one extent or another. 

  i have a number of decorations as does my brother. both of us have sustained injuries in the service to include burns and gunshot wounds, and i was stabbed twice.  so if you want toss around the word "coward" i suggest you go find one.
